Long-Term Care and Rehabilitation

Joseph A. Libby, MD - Medical Director

Joseph Libby, MD, was born in South Philadelphia and raised in South Jersey. A graduate of Bishop Eustace Prep, Saint Joseph’s University, and Robert Wood Johnson Medical School, Dr. Libby trained and practiced at Cooper Hospital for nearly 20 years.

He then went into private practice for five years, specializing in Elder Care, post-hospital care, nursing home care and assisted living.

Dr. Libby is now serving as Medical Director of Jefferson Health New Jersey's Skilled Nursing Facility (SNFist) program, spanning the continuum of Elder Care needs from home, hospital, post-hospital, rehabilitation, assisted living care, and back to home, with the goal of maximizing each individual’s potential for health and functional independence.

Long-Term Care/Nursing Home

Our 111-bed long-term facility features modern private and semi-private rooms for residents, a spacious dining room and a day room that opens onto a lush courtyard. The Center provides a wide range of activities for every interest—from pet therapy to seasonal activities—that encourage independence, social interaction, and comfort.

Sub-Acute Rehabilitation Center

Our 79-bed sub-acute rehabilitation center serves patients who have experienced an acute medical event (i.e. stroke, head trauma, illness) and need a "bridge" between a hospital stay and a return to home, usually lasting less than a month. This two-story center is connected to the existing Jefferson Health Care Center, with an enclosed courtyard between them so residents from both can spend quality time outside. There are both private and semi-private patient rooms and even patients in a shared room enjoy a sense of privacy, as the rooms are designed with separate bedroom space. Each floor has a kitchen and dining area open to both patients and visitors. These kitchen areas are arranged so they may also serve as a training area for patients receiving occupational therapy. Physical, speech and occupational therapy are provided in the second-floor gym. The landscape plan includes a healing garden in the courtyard and the lobby has a modern waterfall element.

The Jefferson Health Care Center has four-star ratings in the category of “Quality Measures,” and “Overall Staffing”  and a five-star rating in “Nursing Staffing” from Medicare.gov/Nursing Home Compare.
