How May I Obtain Copies of My Medical Records?
To get a copy of your medical records, you must complete and sign an Authorization to Release Protected Health Information and return it to us in person. Signatures other than the patients must have documentation of authority to sign. Medical records will not be released without a written authorization. Do I Have to Pay for Copies?
There will be a charge to cover the photocopying costs. This is authorized by the New Jersey Department of Health. Fees of 14 cents per page, plus postage. There is no charge for information requested by or for your physician.
Can You Give Me My Medical Information Over the Phone or By Fax?
Due to the need to protect patient confidentiality, we do not supply information over the phone. In the case of immediate patient care purposes, we can fax records to a physician's office or health care facility upon a signed authorization. Otherwise, copies of medical records will be mailed or held for pickup.
Does My Primary Care Physician Get Copies of My Medical Records?
Your family physician will not automatically receive a copy of your entire record but may receive pertinent information, such as lab results. If you were seen in our emergency department, a copy of your records will be faxed to your primary care physician if you authorized this on the Treatment Consent form during your emergency room visit.
Can I Have Someone Else Pick Up My Medical Records for Me?
Yes, on the signed authorization form, specify that your records can be released to that person. We will confirm your signature on the authorization and we must also be able to confirm the identity of the person picking up your records. Otherwise, we do not allow anyone but the patient to pick up medical records unless they have Power of Attorney.
