Patient Navigator Program

You have enough to do with your everyday responsibilities. When you or a loved one is sick or hurt, aside from the worry that comes with it, there are usually the headaches of dealing with insurance, making appointments, following up, and so on. It's enough to make you feel overwhelmed and out of control. But at Jefferson Health New Jersey, you don't have to.

Patient Navigator Program

We'll Navigate the Way

Let our Jefferson Health New Jersey Nurse Navigator RNs coordinate your care, so all you have to do is focus on getting better. Each Navigator is a highly trained R.N. Navigators are available at our three Emergency Departments (ERs), at all Kennedy Health Alliance medical offices, and at the Cancer Center, where patients have the added support of a full time social worker.
